Instructions:

1. Click on “Select Company” and search for your company from the drop-down

2. Click on “Select Facility” and select your facility (note: sometimes it takes 10-15 seconds to populate facilities)

3. Click on “Select Name” and select your name from the list, you will need your student ID from your certificate or from the field instructor.
